Wellbeing Week template

Children take part in Premier League Wellbeing Stars by completing this template. Parents/carers can share their work with us using the parent online form. Teachers can share their pupils' work with us using the upload form on the challenge page.

There is also an example of a completed template to help children understand how to take on the challenge. 


Premier League Wellbeing Stars certificate

Reward your child with a Premier League Wellbeing Stars certificate. Ask them to think about what their next social action goal is and write it in the box below.
